When you need to file anything with a court, you should always check to see if there is a form for your exact legal problem. There are Judicial Council forms for many civil and criminal cases. Some of these forms are adopted (using them is mandatory) and some are approved (using them is optional, which means you must still provide the information requested on the form, but you can use a different form or just write up the information without using a form).
